Leah-Headshot-1-300x300 | Working at Height: Why Small Mistakes Have Major Consequences | EHS InternationalBy Leah Murphy, Head of Training, EHS International

Working at height remains one of the leading causes of serious workplace injuries and fatalities worldwide.

Despite significant advances in equipment, planning processes and safety management systems, falls from height continue to occur across construction, maintenance, utilities, manufacturing and many other sectors.

What makes working at height particularly concerning is that incidents often result from seemingly minor decisions.

A shortcut.

A missed inspection.

A failure to use equipment correctly.

Small mistakes can have life-changing consequences.

Understanding the Risks

Many people associate working at height with large construction projects or high-rise developments.

In reality, falls can occur from relatively low heights.

Ladders, platforms, roofs, mezzanines and temporary access equipment all present potential risks.

The severity of an injury is not always determined by the height involved.

Factors such as landing surfaces, environmental conditions and the nature of the fall can significantly influence outcomes.

Human Factors and Complacency

One of the greatest challenges in managing work at height is complacency.

Tasks that are performed regularly can become routine.

As familiarity increases, risk perception can decrease.

Workers may begin to:

  • Skip checks
  • Take shortcuts
  • Ignore procedures
  • Underestimate hazards

This is often where incidents begin.

Maintaining awareness and vigilance is essential, particularly during routine activities.

Planning Makes the Difference

Effective work at height safety begins long before anyone leaves the ground.

Proper planning should consider:

  • The task being performed
  • Access requirements
  • Environmental conditions
  • Equipment suitability
  • Emergency arrangements

When planning is overlooked, the likelihood of errors increases significantly.

Competence Matters

Equipment alone does not create safety.

Competent individuals create safety.

Training plays a critical role in helping workers understand:

  • Hazard identification
  • Equipment inspection
  • Safe work practices
  • Fall prevention measures
  • Emergency procedures

Competence allows individuals to recognise changing conditions and make informed decisions when risks arise.

Working at height will always involve a degree of risk.

However, those risks can be effectively managed through planning, competence and strong safety culture.

Most incidents are not caused by a single catastrophic failure.

They are often the result of small decisions that accumulate over time.

That is why attention to detail matters.

Because when working at height, even small mistakes can have major consequences.
